How to apply for Unemployment Insurance Benefit/ File a Claim in New York
- Claims can be made by using the following link "fluidnow".
- The "fluidnow" link can be accessed 24 hours a day, right throughout the week.
- Claims can also be made over the telephone by calling 1-800-204-2418 between the hours of 8:00 a.m. to 5:00 p.m. on weekdays.
- For speedy processing of a claim, the claimant should ensure that they apply for Unemployment Insurance in the first week of unemployment.
- The following information is necessary for filing a claim:
- Social Security number
- New York State driver's license
- Mailing address
- Telephone number
- The names and addresses of all the claimants employers within the last 18 months, including those employers in another state
- Your DD-214 form (if you were in the military within the last
2 years) - Form SF-50 or Form SF-8 and check stubs or W-2 proof of
earnings (if you were a federal employee) - Employer Registration number
The requirements for eligibility in each State;
- Employment in at least two calendar quarters in your base period.
- Minimum earnings in your base period for minimum weekly benefit must be $3,400
- Minimum earnings in high quarter for minimum weekly benefit must be $832
- Minimum earnings in your base year for maximum total benefit must be $28,600
- Minimum earnings in high quarter for maximum total benefit must be $7,150
